Second Level Schools Application
Thank you for your interest in YWAM DP’s second level schools. This application page is for students applying to any of our three second level schools: Bible Core Course, Foundations in Counseling Ministries and Word by Heart. Please follow all the steps in the application process. Once we receive your application, our registrar will contact you. If you have questions, please email firstname.lastname@example.org.
1. Complete the Application
Please take your time and prayerfully complete the application form in its entirety. When complete, please click the submit button at the bottom of the application to send the form to the YWAM DP registrar.
2. Submit your Application Fee
Please submit your application fee right away. You may mail a check from a USA bank to the finance office or make an online payment using your credit/debit card or Pay Pal.
3. Send a Photo
Please send a recent photo of yourself as an e-mail attachment to email@example.com. Photos must be high-resolution ‘head shots’ that have you as the primary subject. Photos will be shared with YWAM DP staff, speakers, and supporters & may be used in YWAM DP advertising, promotional materials or other areas
5. Health Form
The Health Form in its entirety must be completed and sent in prior to an applicant being considered for the program.
If the health form from your DTS is less than 2 years old, you can submit that form instead of going through the process to create a new one.
If your health form is 2 years old or older, you must submit a new one in its entirety. The medical form is in two parts. Part A is to be completed by the applicant and reviewed by a Physician, Physician’s Assistant (PA) or Nurse Practicioner (NP). Part B must be completed by a Physician, PA or NP. Finally, the completed form should be scanned and emailed to the YWAM DP Registrar at firstname.lastname@example.org.
If you do not yet have a valid passport or if your passport will expire within a year of the start date of your lecture phase, please do not delay; take whatever steps are necessary to acquire a valid passport.
